There's not a lot of room for custom design. Yes, it is just a wireframe, but sometimes you have to search for an element that fits what you're thinking of and you can't find it immediately, when it would be easy enough to just draw what you're trying to do manually. Balsamiq is easy to use and to introduce to other teammates without a design background. The ability to drag-and-drop components makes it intuitive for most users! Their component gallery has pretty much all the elements one would need to create wireframes for testing ideas.
