Inverted QR codes are not valid QR codes, this is really a bug with dark reader. I guess it must be quite difficult for a third party solution to determine which bits of a website are background colour and which are important, like QR codes. I will see whether scanning twice, once normal and once inverted, causes problems, but it's a nasty workaround.

Thanks for the suggestion, but I don't think this field should be automatically added because this is just one possible way in which people use {TIMEOTP}. It is also very common to have {TIMEOTP} in the password field instead, or as part of the auto type sequence. For users of KPEnhancedEntryView, it is also possible to obtain the OTP directly from the TimeOtp-Secret field without needing any other field or changes.
