Draft: Create On-call Training issue template

Creating the first iteration of an on-call training for Gitaly SWEs. This will provide an introduction to investigating Gitaly issues in a self-managed environment.

High-level ideas for the content were outlined by @anton in #6751 (comment 2497795238)
