The invention relates to a multifunctional tracheal cannula assembly, and belongs to the field of medical instruments.
Background
When a part of bronchoscopes are involved in an operation, bronchoscopes need to be inserted repeatedly, certain wounds are caused to vocal cords and nasal mucosa, particularly, when tumors are removed by freezing in a bronchial cavity and a frozen lung biopsy is carried out, probes and frozen objects with extremely low temperature can directly contact normal tissues such as the vocal cords, the nasal mucosa and the oral mucosa, and the tissues can be torn and damaged, therefore, tracheal intubation or hard-scope technology is often required when the part of the operation is carried out, the operation can be carried out under the condition of general anesthesia and even muscle relaxation, and the injury of the parts can be caused by the operation, so that the popularization and the application of related technologies are greatly limited. Therefore, there is a need for a tracheal tube that can be easily inserted to isolate the site and avoid injury.
When a part of bronchoscopes intervene in an operation, various operation instruments need to be placed into an airway at the same time, or the part of instruments are too large and cannot be inserted into the airway directly through a working hole passage of the bronchoscope, and the diameter of a conventional tracheal cannula is too small, so that the clinical requirement cannot be met frequently, and the operation can be performed under the general anesthesia condition. This requires a tracheal cannula with a large diameter to facilitate the simultaneous insertion of multiple instruments, and also requires that the cannula be easily inserted.
Large hemorrhage related to bronchoscope operation is the most serious complication of respiratory intervention, and once the large hemorrhage occurs, tracheal intubation is often not available, and once the bronchoscope is pulled out, a large amount of hemorrhage causes visual field loss of the tracheal intubation, and the success rate of the tracheal intubation is low; for a patient with high risk of major hemorrhage, the patient is better prevented by sleeving a tracheal cannula on a bronchoscope in advance, but the existing tracheal cannula is not provided with a breathing machine connecting device once the bronchoscope is sleeved on the tracheal cannula, and the operation is influenced after the bronchoscope is sleeved on the tracheal cannula in the overlong mode, so that inconvenience is brought to work. Thus, there is a need for a tracheal tube with a ventilator connection device, which is of a suitable length, is suitable for fitting over a bronchoscope in advance, and is easy to insert.
The mouth piece is a medical consumable commonly used when a bronchoscope is inserted into the mouth for examination and treatment, can provide an endoscope inserting channel, simultaneously protects the endoscope from being damaged by teeth, and is provided with a hard supporting tube and a baffle fixedly connected with the supporting tube, wherein the supporting tube is exposed out of the mouth, and the inner wall of the supporting tube is a tubular cavity.
The multifunctional bronchoscope sleeve provided by the invention is characterized in that the middle-rear section of the main pipe of the multifunctional bronchoscope sleeve is provided with an inclined pipe which forms an angle with the main pipe and can be connected with a breathing machine. In use, the main pipe part can smoothly pass through the cavity of the mouth piece, but the inclined pipe cannot pass through the mouth piece and is fixed without places to influence the operation, so that a new mouth piece is needed, the insertion of the novel bronchoscope sleeve pipe can be met, the sleeve pipe can be fixed, and the sleeve pipe is prevented from being separated in the bronchoscope operation process.
Disclosure of Invention
The invention aims to provide a multifunctional bronchoscope cannula which is easy to insert, can isolate an airway operation injury part and has a larger caliber, and a mouth piece which has a bifurcation structure at the tail end and can fix the cannula.
In order to achieve the purpose, the invention adopts the following design scheme:
a multifunctional tracheal cannula assembly comprises a cannula assembly and a bite block;
wherein the sleeve part consists of an outer sleeve and an inner core; the inner core penetrates through the inner part of the outer sleeve and is connected with the outer sleeve through a first sealing plug at the tail end of the outer sleeve;
the outer sleeve comprises a main pipe, an inclined pipe, a sacculus catheter and a first sealing plug; the main pipe is a hollow straight pipe, a saccule is fixedly connected to the side wall of the head end of the main pipe in a surrounding manner, and a first sealing plug with an operation hole is bonded to the tail end of the main pipe; the inclined pipe is a hollow straight pipe communicated with the main pipe, the front end of the inclined pipe is fixedly connected to the pipe wall of the main pipe close to the tail end, and the tail end is connected with a breathing machine interface; the balloon catheter is divided into an inner part and an outer part which are connected and communicated, the inner part is arranged in the pipe wall of the main pipe, the outer part is arranged outside the main pipe, the head end inside the inner part is provided with an opening communicated with the inner part of the balloon on the outer side wall of the main pipe, the tail end inside the inner part and the head end outside the outer part are arranged at positions close to the tail end of the main pipe, and the tail end outside the outer part is connected with a catheter plug;
the inner core is a hollow straight pipe with openings at two ends, the length of the inner core is greater than that of the main pipe of the outer sleeve, the part of the head end of the inner core, which exceeds the outer sleeve, is in a circular truncated cone shape, and the tail end of the inner core is bonded with a second sealing plug with an operation hole;
the mouth piece comprises a tubular body and a baffle fixedly connected with the outer side of one end of the tubular body, a slope groove is arranged on the inner side surface of the tubular body and faces the outer side of the tubular body, and the inner surface of the tubular body is connected with the inner surface of the slope groove; the slope-shaped groove is opened on the surface of the baffle and is fixedly connected with the baffle; the height of the slope groove is less than or equal to that of the tubular body; the included angle between the slope surface of the slope groove and the axis of the tubular body is an acute angle.
The length of the main pipe of the outer sleeve is 130-200 mm, the inner diameter is 8-14 mm, and the thickness of the pipe wall is 0.5-1.5 mm; the length of the inclined tube of the outer sleeve is 30-50 cm, the outer diameter is 7-9 mm, and the thickness of the tube wall is 0.5-1.5 mm; the main pipe of the outer sleeve and the inclined pipe form an acute angle of 30-50 degrees.
Preferably, the length of the main pipe of the outer sleeve is 150mm, the inner diameter is 14mm, and the thickness of the pipe wall is 1 mm; the length of the inclined tube of the outer sleeve is 40cm, the outer diameter is 7mm, the thickness of the tube wall is 1mm, and an acute angle formed by the main tube of the outer sleeve and the inclined tube is 30-45 degrees.
The tail end inside the balloon catheter and the head end outside the balloon catheter are arranged at the position 10mm away from the tail end of the main pipe.
The length of inner core is 150mm ~ 220mm, and the external diameter of inner core slightly is less than the main pipe internal diameter of outer tube, reduces to 6.5 ~ 7.5mm in the external diameter of round platform portion gradually, and the external diameter of preferred round platform portion reduces to 7mm gradually, and the inner core of being convenient for is inserted in the trachea along the bronchoscope, avoids damaging the glottis.
And the outer sleeve of the sleeve piece and the first sealing plug and the second sealing plug at the tail end of the inner core are both fixedly bonded at the tail end of the sleeve. The first sealing plug at the tail end of the outer sleeve is made of latex, the middle of the first sealing plug is provided with an operation hole capable of expanding and contracting axially, the diameter of the first sealing plug is 4-6mm, and the first sealing plug not only can be conveniently inserted into a bronchoscope or other surgical instruments, but also can keep a relatively closed airway loop to carry out auxiliary ventilation of a breathing machine. The second sealing plug of the inner core is made of latex, an operation hole capable of expanding and contracting axially is formed in the middle of the second sealing plug, the diameter of the second sealing plug is 2-4mm, and the second sealing plug can be inserted into a bronchoscope through the operation hole and fixes the cannula assembly on a bronchoscope body.
The main pipe of the outer sleeve of the sleeve piece is a main channel for operation, is used for an insertion channel of a bronchoscope and other operation instruments, and is also a sleeve for protecting structures such as glottis, oral cavity, teeth and the like; the inclined tube is a joint connected with the breathing machine, is communicated with the main tube and is used for ventilation in the operation; when the respirator ventilates, the saccule is inflated through the saccule conduit to close the air passage.
The seaming device is provided with a hoop structure at the joint of the tubular body and the slope groove. In order to increase the clamping effect, one or more clamping structures can be fixedly arranged in the slope-shaped groove.
The clamp structure of the mouth piece is a convex edge, and can also be a binding band type clamp structure or a buckle type clamp structure.
And making a cross section along the direction parallel to the baffle of the seaming device, and intersecting with the slope surface of the slope-shaped groove, wherein the line of the slope surface of the slope-shaped groove is a straight line, a broken line, an arc line, a regular or irregular polygonal line. The slope surface is matched with the shape of the lateral branch structure of the matched instrument, for example, when the lateral branch structure is a circular tube, the line of the slope surface of the slope groove is arc-shaped, and better matching and fitting are realized.
The cross section of the inner cavity of the tubular body of the mouth piece is circular or elliptical, the diameter is 1 cm-2 cm, and the height of the tubular body is 2-4 cm; the width of the slope groove of the seaming device is 0.5 cm-1 cm, and the height is 0.5 cm-4 cm; the included angle between the slope surface of the slope groove of the mouth piece and the axis of the tubular body is 30-60 degrees, preferably 35-45 degrees.
Furthermore, two sides of the baffle of the mouth piece are provided with fixing buckles. The fixing buckle is used for fixing the frenulum, is convenient for a patient to wear, and prevents the mouth piece from falling off.
The using method of the invention is as follows:
1. the mouth piece is fixed between the upper and lower teeth of a patient and is fixed on the head of the patient by a lace passing through a fixing buckle, and the inner cavity of the mouth piece is matched with the structure at the joint of a main pipe at the tail end of the outer sleeve and an inclined pipe so as to fix the multifunctional tracheal sleeve;
2. fixing the inner core of the sleeve (sleeved in the main tube body of the tracheal sleeve) at the proximal end of the endoscope body of the bendable bronchoscope;
3. inserting the bronchoscope into the trachea and the bronchus along the cleft for routine examination;
4. when high-risk operation is performed or large bleeding related to bronchoscope operation occurs, the sleeve piece is inserted into a trachea along the bronchoscope, and after the position of the sleeve piece is determined to be correct through observation of the bronchoscope, the sleeve piece is fixed through the mouth piece, namely, an inclined tube of the sleeve piece is pushed into a slope groove of the mouth piece and is fixed in the slope groove in a hoop structure;
5. withdrawing the bronchoscope and pulling out the inner core;
6. and connecting a pipeline interface of the respirator with a respirator interface at the tail end of the inclined tube of the outer sleeve to perform auxiliary ventilation of the respirator.
7. The bronchoscope and other instruments are reinserted into the main tube of the outer cannula for surgery.
The invention has the advantages that: the invention is composed of a sleeve piece and a mouth piece which are independently subpackaged and combined for use. The inner diameter of the sleeve piece is large, so that the bronchoscope and other instruments can be conveniently guided to be inserted into the air passage for operation; the length of the sleeve piece is proper, the sleeve piece is sleeved on the bronchoscope, the operation of the bronchoscope is not influenced, and the sleeve piece can be inserted into an trachea to protect vocal cords and can carry out effective ventilation support; the sleeve piece is provided with an inner core, the head end part is in a round table shape, the outer diameter is small, the vocal cords are convenient to pass through the glottis, the vocal cords are not easy to damage, and the tolerance of a patient can be improved; the inclined tube can be connected with a breathing machine for ventilation, so that synchronous anesthesia and breathing treatment can be flexibly and conveniently implemented in the process of performing an operation in an airway, and the continuity, convenience and safety of operation are ensured. The mouth piece has simple and reasonable structure and convenient use, can not only meet the insertion of a casing piece, but also fix the inclined tube structure at the tail end, and effectively prevent the casing piece from being separated in the operation process of the bronchoscope.

Detailed Description
Example 1
Referring to fig. 1 to 9B, a multifunctional tracheal cannula assembly comprises a cannula assembly 1 and a mouthpiece 2;
wherein the sleeve member 1 consists of an outer sleeve 11 and an inner core 12; the inner core 12 penetrates through the inner part of the outer sleeve 11 and is connected with the outer sleeve 11 through a first sealing plug 115 at the end of the outer sleeve 11;
the outer sleeve 11 comprises a main tube 111, an inclined tube 112, a balloon 113, a balloon catheter 114 and a first sealing plug 115; the main pipe 111 is a hollow straight pipe, the side wall of the head end of the main pipe 111 is fixedly connected with a saccule 113 in a surrounding manner, and the tail end of the main pipe 111 is bonded with a first sealing plug 115 with an operation hole 1151; the inclined tube 112 is a hollow straight tube communicated with the main tube 111, the front end of the inclined tube is fixedly connected to the tube wall of the main tube 111 close to the tail end, and the tail end is connected with a breathing machine interface 1121; the balloon catheter 114 is divided into an inner tube 1141 and an outer tube 1142 which are connected and communicated, the inner tube 1141 is arranged in the tube wall of the main tube 111, the outer tube 1142 is arranged outside the main tube 111, the head end of the inner tube 1141 is provided with an opening 11411 communicated with the inner part of the balloon 113 on the outer side wall of the main tube 111, the tail end 11412 of the inner tube 1141 and the head end of the outer tube 1142 are arranged on the tube wall close to the tail end of the main tube 111, and the tail end of the outer tube 1142 is connected with a catheter plug;
the inner core 12 is a hollow straight pipe with openings at two ends, the length of the inner core 12 is greater than that of the main pipe 111 of the outer sleeve, the side wall of the part of the head end of the inner core 12, which exceeds the outer sleeve, is in a circular truncated cone shape 121, and the tail end of the inner core is fixedly bonded with a second sealing plug 122 with an operation hole 1221;
the length of the main pipe 111 of the outer sleeve 11 is 150mm, the inner diameter is 14mm, and the thickness of the pipe wall is 1 mm; the length of the inclined tube 112 of the outer sleeve 11 is 40cm, the outer diameter is 7mm, the thickness of the tube wall is 1mm, the acute angle alpha formed by the main tube of the outer sleeve and the inclined tube is 45 degrees, the tail end of the outer sleeve is bonded with a first sealing plug 115 made of latex, and the diameter of the operation hole 1151 which can axially expand and contract is arranged in the middle of the outer sleeve is 5 mm.
The distal end 11411 of the tube interior 1141 and the tip end of the tube exterior 1142 of the balloon catheter 114 are provided at positions 10mm from the distal end of the main tube 111.
The length of the inner core 12 is 170mm, the outer diameter of the inner core 12 is slightly smaller than the inner diameter of the main pipe 111 of the outer sleeve and is 13.5mm, the outer diameter of the circular table part 121 is gradually reduced to 7mm, the tail end of the inner core is adhered with a second sealing plug 122 made of latex, and the diameter of an operation hole 1221 which can be expanded and contracted axially is 3 mm.
The mouthpiece 2 comprises a tubular body 22 and a baffle 21 fixedly connected with the outer side of one end of the tubular body, and fixing buckles 23 are arranged on two sides of the baffle. A slope groove 24 is arranged on the inner side surface of the tubular body 22 and faces the outer side of the tubular body 22, and the inner surface of the tubular body 22 is connected with the inner surface of the slope groove 24; the cross-sectional shape of the tubular body lumen 22 is circular, with a diameter of 1.6cm and a height of 4 cm; the width W of the slope-shaped groove 24 is 0.8cm, and the slope-shaped groove 24 is opened on the surface of the baffle plate 21 and is fixedly connected with the baffle plate 21; the height H of the groove 24 is less than the height of the tubular body 22 and is 2 cm; the included angle beta between the slope surface of the slope groove 24 and the axis of the tubular body is 45 degrees.
A clamping structure 25 is arranged at the joint of the tubular body 22 and the slope groove 24, and the clamping structure is a convex rib.
And making a cross section along the direction parallel to the baffle 21, and intersecting with the slope surface of the slope-shaped groove 24, wherein the line of the slope surface of the slope-shaped groove 24 is an arc line.
The using method of the invention is as follows:
1. the mouth piece 2 is fixed between the upper and lower teeth of the patient and is fixed on the head of the patient by a lace passing through a fixing buckle 23, and the inner cavity of the mouth piece is matched with the structure at the joint of a main pipe 111 and an inclined pipe 112 at the tail end of an outer sleeve for fixing the sleeve part 1;
2. fixing the inner core 12 of the sleeve member 1 (sleeved in the main tube 111 of the tracheal sleeve member) to the proximal end of the body of the flexible bronchoscope 3;
3. inserting the bronchoscope 3 into the trachea and the bronchus along the clenching device 2 for routine examination;
4. when high-risk operation is performed or large bleeding related to bronchoscope operation occurs, the cannula part 1 is inserted into a trachea along the bronchoscope 3, after the position of the cannula part 1 is determined to be correct through observation of the bronchoscope 3, the cannula part 1 is fixed through the mouth piece 2, namely, the inclined tube 112 of the cannula part 1 is pushed into the slope-shaped groove 24 of the mouth piece, and is fixed in the slope-shaped groove 24 through the hoop structure 25;
5. withdrawing the bronchoscope 3 and extracting the inner core 12;
6. the ventilator tube interface 4 is connected 1121 with the ventilator interface at the end of the inclined tube 112 of the outer cannula for ventilator assisted ventilation.
7. The bronchoscope 3 and other instruments are reinserted into the main tube 111 of the outer sleeve 11 for surgery.
Example 2
Referring to fig. 10, the difference from embodiment 1 is that the height H of the slope groove 24 of the bite piece 2 is equal to the height of the tubular body 22.
