Abstract
A surgical retraction apparatus frame 11 having a central annulus and being substantially wider than it is thick. The frame is malleable in order to conform to the shape of the incision. The frame may be inelastic deformable, and made of 304 stainless steel. The frame 11 is substantially planar and may be rectangular or square with rounded edges. The frame may include perforations 12 to receive an elongate elastic stay 14. The stay 14 is drawn through the perforations and has a cross section which decreases under tension to allow penetration of the perforation. The stay may have detents to retain the stay, a hook to engage the wound, and a tapered end to attach to the ring. The stay is claimed both separately and in combination with the frame.

Surgical Retraction Apparatus
This invention relates to surgical retraction apparatus.
Surgical retraction apparatus is used for opening and maintaining open an incision to allow access for surgical procedures. Such apparatus may comprise an anchor plate adapted to be located around an incision site to which may be anchored stays comprising hooks placed at the edges of the incision and connected to the anchor plate, by which the edges may be drawn back towards the anchor ring and there retained.
The anchor plate must hold its shape under the forces in the stays, and is generally of rigid construction. In order to accommodate different incision sites, for example in the thorax, abdomen, spine, and limb, anchor plates of many different shapes and sizes must be provided.
The present invention provides a surgical retraction apparatus that can be realised in fewer different shapes and sizes to meet the requirements of different incision sites.
The invention comprises a surgical retraction apparatus comprising an anchor plate comprising an annulus having a width substantially greater than its thickness and of such material that it is deformable away from a flat configuration to conform to the contours of an incision site.
While deformation may take place elastically, which might involve some measure of retention of the elastically deformed plate, as by stays affixed to drapes or other local structure, inelastic deformation, which may be effected prior to or during application to the incision site, avoids the need for such retention.
The material may be steel, which may be 304 stainless, annealed, non-magnetic austenitic steel.
The width to thickness ratio may be between 15:1 and 40:1, for example 20:1. The width may for example be 10mm and the thickness 0.5mm.
The annulus may be a rounded rectangle, and may be a rounded square. A rounded rectangle may for example have an internal major axis of 180mm and an internal minor axis of 130mm. A rounded square may have internal axes of 140mm.
The annulus may have perforations around it, which may be spaced apart by between 10 and 30mm, for example, 20mm. The perforations, serving as attachments for the stays may be 2.5mm across. Corner lugs may be provided, which may have like perforations, which may serve as attachments for stays anchored to drapes.
While anchor plates according to the invention may be used with conventional stays, the invention also provides a stay particularly adapted for use with the anchor plates comprising a flexible tie adapted to be drawn through a perforation in the plate having lengthwisespaced detents retaining the needle against withdrawal. The detents may be spaced at 10mm intervals. The tie may be of a silicone material. The stay may be tapered at one end for ease of threading through a perforation and will have an incision-engaging hook at the other end.
The tie may be elastically extensible, and contract in cross-section under tension. It may then be relatively easy to pull through an anchor plate perforation while under tension through the hook being engaged in the incision, but, once through, and tension relaxed, the untensioned cross-section will be larger than that of the perforation and will not be pulled back through the perforation.
In use, the anchor plate is placed around an incision and deformed to conform to the contours around the incision site, then the stays are hooked into the incision and threaded through perforations in the anchor plate, and pulled through until the incision is opened as far as the surgeon requires.
Embodiments of surgical retraction apparatus according to the invention will now be described with reference to the accompanying drawings, in which:
Figure 1 is a plan view of a first embodiment of anchor plate;
Figure 2 is a plan view of a second embodiment of anchor plate, in use;
Figure 3 is a view of a stay used with the anchor plates of Figures 1 and 2; and
Figure 4 is a diagrammatic view of an anchor plate conformed to the contours of an incision site.
The drawings illustrate a surgical retraction apparatus comprising an anchor plate 11 comprising an annulus having a width substantially greater than its thickness and of such material that it is deformable away from a flat configuration to conform to the contours of an incision site.
The material is 304 stainless, annealed, non-magnetic austenitic steel.
The width to thickness ratio is 20:1, the width being 10mm and the thickness 0.5mm. Other ratios, widths and thicknesses will be satisfactory
The annulus of Figure 1 is a rounded rectangle, and that of Figure 2 a rounded square. A rounded rectangle has an internal major axis A of 180mm and an internal minor axis B of 130mm. A rounded square has internal axes A of 140mm. Because of the malleability of the austenitic steel, these two sizes will be found to cover many surgical procedures.
The annulus has perforations 12 around it, which are spaced apart by 20mm. The perforations, serving as attachments for the stays, are 2.5mm across.
The annulus of Figure 1 has corner lugs 13, which have like perforations 12, which can serve as attachments for stays anchored to drapes.
Figure 2 illustrates the anchor plate 11 in use at an incision site IS with the incision retracted and held by stays 14.
While anchor plates 11 may be used with conventional stays, Figure 3 illustrates a stay 14 particularly adapted for use with the anchor plates 11, comprising a flexible tie 15 adapted to be drawn through a perforation 12 in the plate 11 having lengthwise-spaced detents 16 retaining the tie 15 against withdrawal. The detents 16 are spaced at 10mm intervals. The tie 15 is of a silicone material, and is tapered at one end 15a for ease of threading through a perforation 12, and has an incision-engaging hook 17 at the other end.
The tie 15 is elastically extensible, and contracts in cross-section under tension. It is then relatively easy to pull through an anchor plate perforation 12 while under tension through the hook being engaged in the incision, but, once through, and tension relaxed, the untensioned cross-section will be larger than that of the perforation 12 and will not be pulled back through the perforation.
In use, as illustrated in Figure 3, the anchor plate 11 is placed around an incision site IS and deformed to conform to the contours around the incision site, Figure 4, then the stays 14 are hooked into the incision and threaded through perforations 12 in the anchor plate 11, and pulled through until the incision is opened as far as the surgeon requires. The ties 15 may be doubled back when pulled through and threaded through a spare perforation 12 to confirm retention and help keep the operating site tidy.
Claims (24)
1 A surgical retraction apparatus comprising an anchor plate comprising an annulus having a width substantially greater than its thickness and of such material that it is deformable away from a flat configuration to conform to the contours of an incision site.
2 Apparatus according to claim 1, in which the plate is inelastically deformable.
3 Apparatus according to claim 1 or claim 2, in which the material is steel.
4 Apparatus according to claim 3, in which the steel is 304 stainless, annealed, nonmagnetic austenitic steel.
5 Apparatus according to any one of claims 1 to 4, in which the width to thickness ratio is between 15:1 and 40:1
6 Apparatus according to claim 5, in which the width to thickness ratio is 20:1.
7 Apparatus according to claim 6, in which the width is be 10mm and the thickness 0.5mm.
8 Apparatus according to any one of claims 1 to 7, in which the annulus is a rounded rectangle.
9 Apparatus according to claim 8, in which the rounded rectangle has an internal major axis of 180mm and an internal minor axis of 130mm.
10 Apparatus according to claim 8, in which the annulus is a rounded square.
11 Apparatus according to claim 10, in which the rounded square has internal axes of 140mm.
12 Apparatus according to any one of claims 1 to 11, in which the annulus has perforations around it.
13 Apparatus according to claim 12, in which the perforations are spaced apart by between 10 and 30mm.
14 Apparatus according to claim 13, in which the perforations are spaced apart by 10mm.
15 Apparatus according to any one of claims 12 to 14, in which the perforations are between 2mm and 3mm across.
16 Apparatus according to claim 15, in which the perforations are 2.5mm across.
17 Apparatus according to any one of claims 1 to 16, in which the annulus has corner lugs having like perforations.
18 Apparatus according to any one of claims 1 to 17, comprising a stay adapted for use with the anchor plates comprising a flexible tie adapted to be drawn through a perforation in the plate having lengthwise-spaced detents retaining the stay against withdrawal.
19 Apparatus according to claim 18, in which the detents are spaced at 10mm intervals.
20 Apparatus according to claim 19, in which the tie is of a silicone material.
21 Apparatus according to any one of claims 18 to 20, the stay being tapered at one end for ease of threading through a perforation and having an incision-engaging hook at the

22 A stay for use with an anchor in a surgical retraction system in which the anchor plate has perforations through which the stay is threaded, the stay comprising an elastically extensible tie which contracts in cross-section under tension so as to be capable of being readily drawn through a perforation of the anchor when under tension on the side of the
10 perforation remote from the side from which it is pulled, but which cannot readily be pulled back through the perforation when not under tension.
23 A stay according to claim 22, having lengthwise-spaced detents retaining the stay against withdrawal.
24 A stay according to claim 23, in which the detents are spaced at 10mm intervals.
15 25 A stay according to any one of claims 22 to 24, in which the tie is of a silicone material.
