Abstract
【解決手段】 クランピング機構と関連超音波エンドエフェクターの協同作業で組織の切断、凝固および固定が起こるような構造にする。上記クランピング機構とエンドエフェクターの選択的な割出し回転位置決めを、この装置のクランプ駆動機構の中に移動止め機構を組み込むことで達成する。このような構造配置にし、この装置のウエイブガイドに関連超音波駆動装置を有効連結させてそれが上記クランピング機構と一緒に回転するようにすると、この装置の細長い部分をこの装置のハウジングに関して選択的に回転可能なように位置させることができる。

Description
(これを上記動作レバーに有効連結させる)とドライブカラー(drive collar)(これを上記作動部材に共同回転するように取り付ける)を含める。このドライブヨークとドライブカラーを、上記動作レバーとドライブヨークの旋回移動を通して上記ドライブカラーと作動部材の往復運動が起こるように噛み合わせる。
うに、本装置を外科装置の超音波駆動装置に関連させて用いることが考えられ、上記駆動装置から出る超音波エネルギーによって本クランプコアギュレーター装置を所望通り超音波作動させる。本発明の原理を具体化したクランプコアギュレーター装置の構造を使い捨て使用でない使用に適するような構造にすることができそしてそれを関連超音波駆動装置と一体化させる(取り外し可能でないように)ことも可能であることは理解されるであろう。しかしながら、この装置を1人の患者に対して一度のみ使用するように本クランプコアギュレーター装置と関連超音波駆動装置を取り外し可能なように連結させるのが現在のところ好適である。
エンドエフェクターの鋭利さ、およびエンドエフェクターと組織の間の連結度合に伴って変わる。
、最も好適には第一共振子92を303ステンレス鋼で加工しかつ第二共振子94を7075−T651アルミニウムで加工する。この第一および第二共振子92および94に持たせる長さは数多くの変数で決定され、そのような変数には変換部分90の長さ、共振子92および94で用いる材料が伝達する音の速度、そして変換器アセンブリ40で望まれる基本的な振動数f0が含まれる。この第二共振子94をそれの基部末端から遠方末端に向かって内側に先を細くすることで速度変換器として機能させて超音波振動幅を増幅させることも可能である。
結具または溶接連結で行うことも可能である。上記音響アセンブリに組織による負荷がかかっていない時にこの音響アセンブリが好適な共振振動数f0に調音されるように、このエンドエフェクターの遠方末端をアンチノード近くに位置させる。このエンドエフェクターの遠方末端の構造を、上記変換器アセンブリにエネルギーを前以て決めておいた振動数f0で与えると上記遠方末端が例えば約10−500ミクロン(ピークからピーク)の範囲、好適には約10から約100ミクロンの範囲で縦方向に動くような構造にする。

上記外側管状被覆物内に往復運動可能なように位置していてそれと一緒に上記ハウジングに関して回転する内側の作動部材;
上記外側管状被覆物の上記遠方末端の遠位に伸びているエンドエフェクターを有していて上記外側管状被覆物内に位置する超音波ウエイブガイド;
上記外側管状被覆物の上記遠方末端に旋回し得るように取り付けられていて上記エンドエフェクターに関して旋回移動することで組織をクランプアームと上記エンドエフェクターの間に固定するクランプアームであって、上記作動部材の往復運動によって上記クランプアームが上記エンドエフェクターに関して旋回移動するように上記作動部材に有効連結しているクランプアーム;
上記ハウジングに旋回可能なように取り付けられている動作レバー、および上記動作レバーの旋回移動で上記作動部材が往復運動することで上記クランプアームが旋回移動するように上記動作レバーと上記作動部材を相互連結させている手段;
を含み、ここで、上記相互連結させている手段は上記作動部材が上記動作レバーに関して回転することを容認するものでありかつ上記作動部材および管状外側被覆物が上記ハウジングに関して起こす回転移動を割出す移動止め手段を含む、

2. 上記相互連結させている手段が、共同回転するように上記作動部材に連結しているドライブカラー、および上記ドライブカラーに噛み合う上記動作レバーに連結しているドライブヨークを含む、
第1項記載の超音波外科クランプ装置。
3. 上記ドライブカラーが、間隔を置いて位置する一対のドライブフランジを含み、上記ドライブヨークが上記ドライブフランジ間にそれらと噛み合うように位置している、
第2項記載の超音波外科クランプ装置。
4. 上記ドライブヨークが、上記ドライブカラーと直径方向で噛み合い得る一対のヨークアームを含む、
第2項記載の超音波外科クランプ装置。
5. 上記移動止め手段が、上記ドライブヨーク上に少なくとも1個の移動止めを含み、そして上記ドライブカラーが、上記作動部材および上記外側被覆物が上記ハウジングに関して割出し回転を起こすように上記移動止めと噛み合ってそれと協同作業する移動止め受け表面を限定している、

6. 上記外側被覆物に取り付けられていて上記外側被覆物および上記作動部材の割出し回転を起こさせる回転ノブ、
を含む第1項記載の超音波外科クランプ装置。
7. 超音波外科クランプ装置であって、
ハウジング;
上記ハウジングに回転可能なように連結している基部末端と遠方末端を有する外側管状被覆物;
上記外側被覆物に取り付けられていてそれを上記ハウジングに関して回転させる回転ノブ、
上記外側管状被覆物内に往復運動可能なように位置していてそれと一緒に上記ハウジングに関して回転する内側の管状作動部材;
上記外側管状被覆物の上記遠方末端の遠位に伸びているエンドエフェクターを有していて上記外側管状被覆物内に位置する超音波ウエイブガイド;
上記外側管状被覆物の上記遠方末端に旋回し得るように取り付けられていて上記エンドエフェクターに関して旋回移動することで組織をクランプアームと上記エンドエフェクターの間に固定するクランプアームであって、上記作動部材の往復運動によって上記クランプアームが上記エンドエフェクターに関して旋回移動するように上記作動部材に有効連結しているクランプアーム;
上記ハウジングに旋回可能なように取り付けられている動作レバー、および上記動作レバーの旋回移動で上記作動部材が往復運動することで上記クランプアームが旋回移動するように上記動作レバーと上記作動部材を相互連結させているクランプ駆動機構;
を含み、ここで、
上記クランプ駆動機構が、共同回転するように上記作動部材に連結しているドライブカラー、および上記ドライブカラーに噛み合う上記動作レバーに連結しているドライブヨークを含むが、それらの間の相対的回転を容認し、そしてここで、上記駆動機構が、上記ドライブカラー、作動部材および上記管状被覆物が上記ドライブヨークおよび上記ハウジングに関して起こす回転移動を割出す移動止め機構を含む、

11. 上記移動止め受け表面が、周囲方向に間隔を置いて位置する歯を複数限定していて、上記歯がそれらの間に上記移動止めを受け取る半径方向のへこみを限定している、

12. 約5から20インチ・オンス未満の相対的回転トルクに耐える上記ヨークアームによって上記移動止めが上記へこみの中に片寄る、

Claims (1)
- 超音波外科クランプ装置であって、
ハウジング;
上記ハウジングに回転可能なように連結している基部末端と遠方末端を有する外側管状被覆物;
上記外側管状被覆物内に往復運動可能なように位置していてそれと一緒に上記ハウジングに関して回転する内側の作動部材;
上記外側管状被覆物の上記遠方末端の遠位に伸びているエンドエフェクターを有していて上記外側管状被覆物内に位置する超音波ウエイブガイド;
上記外側管状被覆物の上記遠方末端に旋回し得るように取り付けられていて上記エンドエフェクターに関して旋回移動することで組織をクランプアームと上記エンドエフェクターの間に固定するクランプアームであって、上記作動部材の往復運動によって上記クランプアームが上記エンドエフェクターに関して旋回移動するように上記作動部材に作動的に連結しているクランプアーム;
上記ハウジングに旋回可能なように取り付けられている動作レバー、および上記動作レバーの旋回移動で上記作動部材が往復運動することで上記クランプアームが旋回移動するように上記動作レバーと上記作動部材を相互連結させている手段;
を含み、上記相互連結させている手段は上記作動部材が上記動作レバーに関して回転することを容認するものでありかつ上記作動部材および管状外側被覆物が上記ハウジングに関して起こす回転移動を指示するための移動止め手段を含み、
ここで、上記相互連結されている手段が、周囲に間隔を置いて位置するくぼみを限定する複数の歯を限定している移動止め受け表面を限定する、共同回転するように上記作動部材に連結しているドライブカラーと、上記動作レバーに連結しており少なくとも一つの移動止めを有するドライブヨークとを含んでなり、上記くぼみが、上記作動部材および管状外側被覆物が上記ハウジングに関して起こす回転移動を指示するための上記少なくとも1つの移動止めを受けるように適合されており、
相互連結されている手段が、鉗子アームとウエイブガイドの対応する回転を可能にする、
