Abstract
Description
本願は、米国特許出願第11/674,463号(2007年2月13日出願)の優先権の利益を主張する。この出願は、本明細書において参照により援用される。
本開示は、概して、医療機器に関し、より具体的には、血管の電気的刺激のためのシステムおよび方法に関する。
DI=(dA/A)/dPx1000
式中、Aは、拡張期管腔面積であり、dAは、最小管腔面積と最大管腔面積との差であり、dPは、パルス圧力である。また、大動脈DIの増加は、体循環により血液を前方に推進する際に有益である。加えて、心室および冠血管系間の連結が、大動脈DIの増加によって改善可能になる。図10は、種々の実施形態に従って適用された電気的刺激療法に関するグラフ図である。電気的刺激は、1Hzおよび16Hzならびに0.5ミリ秒および50ミリ秒のパルス幅において、10ボルトの刺激波形を使用する制御条件に対して、伸展性を増加させることが示されている。他の電気的刺激波形を使用して、本開示の範囲を逸脱することなく、伸展性を増加させることが可能である。
Claims (26)
- 血管との電気接触を有する少なくとも1つの電極と、
該電極に電気的に接続される刺激回路であって、該血管に刺激を提供するように適合される回路と、
該回路に接続される制御器であって、該血管の収縮および該血管の拡張を含む血管療法に選択的に及ぼすように、該刺激回路の周波数パラメータおよび電圧パラメータを選択するように適合される制御器と
を備える、システム。 - 前記刺激回路および制御器は、埋め込み型医療機器(IMD)内に含まれる、請求項1に記載のシステム。
- 前記制御器に接続されるセンサをさらに備える、請求項1に記載のシステム。
- 前記IMDは、前記センサから受信した信号に基づいて、療法を開始または変更する、請求項3に記載のシステム。
- 前記IMDは、無線テレメトリーを介して外部ソースから受信した信号に基づいて、療法を開始または変更する、請求項1に記載のシステム。
- 前記制御器は、血管拡張をもたらすために、第1の所定の周波数および第1の所定の電圧ならびに第1の所定のデューティサイクルで、第1の電気的刺激療法を血管に適用し、血管収縮をもたらすために、第2の所定の周波数および第2の所定の電圧ならびに第2の所定のデューティサイクルで、第2の電気的刺激療法を血管に適用し、および血管マッサージを提供するために、拡張および収縮のタイミングおよび持続時間を制御するように適合される、請求項1に記載のシステム。
- 前記制御器は、血管コンプライアンスを増加させるために、前記血管マッサージを調整するように適合される、請求項6に記載のシステム。
- 前記制御器は、前記第1の電気的刺激療法を下行大動脈に適用する、請求項6または7に記載のシステム。
- 少なくとも1つの生理的パラメータを検知するためのセンサをさらに備え、
前記拡張および収縮のタイミングおよび持続時間は、該検知されたパラメータに基づいて調整される、請求項6から8のいずれか1項に記載のシステム。 - 前記センサは、血圧を検知するように適合される、請求項9に記載のシステム。

- 血管を拡張させるために該血管の平滑筋を電気的に刺激する手段であって、該平滑筋の緊張を減少させるために、所定の周波数および所定の電圧ならびに所定のデューティサイクルで該平滑筋を刺激する手段を含む手段と、
生理的パラメータを検知する手段と、
該検知された生理的パラメータに基づいて、該血管に対する該電気的刺激を調整する手段と
を備える、システム。 - 前記平滑筋を電気的に刺激する手段は、約1Hzの周波数で刺激を印加する手段を含む、請求項12に記載のシステム。
- 前記平滑筋を電気的に刺激する手段は、約70ボルトの電圧で刺激を印加する手段を含む、請求項12または請求項13に記載のシステム。
- 前記所定のデューティサイクルは、連続デューティサイクルを含む、請求項12から14のいずれか1項に記載のシステム。
- 前記平滑筋を電気的に刺激する手段は、埋め込み型機器を使用して刺激を印加する手段を含む、請求項12から15のいずれか1項に記載のシステム。
- 前記埋め込み型機器を使用して刺激を印加する手段は、スタンドアロン型狭心症療法機器を含む、請求項16に記載のシステム。
- 前記埋め込み型機器を使用して刺激を印加する手段は、スタンドアロン型高血圧療法機器を含む、請求項16に記載のシステム。
- 前記埋め込み型機器を使用して刺激を印加する手段は、心臓機器を含む、請求項16に記載のシステム。
- 血管収縮をもたらすために、少なくとも16Hzの周波数かつ少なくとも10ボルトの電圧レベルで、血管に電気的刺激を印加する手段と、
療法を調節するために、収縮のタイミングおよび持続時間を制御する手段と
を備える、システム。 - 前記療法を調節するために、収縮のタイミングおよび持続時間を制御する手段は、血圧を調節するために、収縮のタイミングおよび持続時間を制御する手段を含む、請求項20に記載のシステム。
- 前記電気的刺激を印加する手段は、少なくとも70ボルトの電圧レベルで刺激を印加する手段を含む、請求項20または21に記載のシステム。
- 少なくとも1つの生理的パラメータを検知する手段をさらに備え、
前記収縮のタイミングおよび持続時間は、該検知されたパラメータに基づいて調整される、請求項20から22のいずれか1項に記載のシステム。 - 前記生理的パラメータを検知する手段は、血圧を検知する手段を含む、請求項23に記載のシステム。
- 前記電気的刺激を印加する手段は、検知された血圧が第1の閾値を上回って上昇する場合に、刺激を印加するように適合され、検知された血圧が第2の閾値を下回って低下する場合に、刺激を停止するように適合される、請求項24に記載のシステム。
- 前記第1の閾値および第2の閾値は、プログラム可能なパラメータである、請求項25に記載のシステム。
