Description
1.2D頭部X線像を取得し、それらについて測定を行い(後者の工程は「2D頭部X線像のトレース」と呼ばれている)、
2.患者顔面について重要な距離を直に測定し、
3.歯列の石膏模型を製作するために上下歯列弓の印象を採得する。患者の実際の咬合を知って、上下歯列の石膏模型を関係付けるために、ワックスバイトによる位置合わせを行い、両石膏模型を咬合器にセットし、
4.段階1と段階2の結果並びに患者の臨床的評価による追加の情報に基づき、咬合が最適化されるよう石膏モデルの位置を直す。この段階で、石膏モデルを切削することもある。
−当人の歯列の印象を採得する段階と、
−当人が印象を装着している状態で、当人の頭部の第1の走査画像を撮影する段階と、
−印象単独の第2の走査画像を撮影する段階と、
−それら走査画像を組み合わせる段階と、
−組み合わされた走査画像から形状情報を引き出す段階と、を備えている。
1.患者の歯列を、頭部の画像データ並びに随意的には同画像ボリュームから取り出したセグメント化された表面部分とを組み合わせた詳細な視覚化。このデータセットを拡張モデルと呼ぶ。
3.計画された通りに手術を行なうのに有効な術中支援。
1.歯列を、随意的には軟組織のプロフィールを乱すこと無く、詳細に視覚化できるようにする画像取得プロトコル。
1.拡張モデルを得る際の患者取り扱いは、単純容易且つ迅速でなくてはならず(これは、少なくとも従来の印象採得及び石膏モデル製作並びに取り扱いと同程度に素早くという意味である)、
2.手術(及び術後)道具の生成は、(半)自動的で安価な処理手順でなくてはならない。
1.印象走査画像(4)を患者+印象走査画像(2)に対しボクセルベースで剛直的に位置合わせし(即ち、位置と向きを整列させ)、
2.詳細な印象走査画像(4)を顎顔面複合走査画像に画像融合し、拡張モデルを得る。

2.患者+印象走査画像(2)の上顎を患者走査画像(3)に上顎に対しボクセルベースで剛直的に位置合わせし、
3.患者+印象走査画像(2)の下顎を患者走査画像(3)の下顎に対しボクセルベースで剛直的に位置合わせし、
4.詳細な印象走査画像(4)を顎顔面複合走査画像に画像融合し、拡張モデルを得る。
頬顎異常矯正手術の計画立案
仮想骨接合術の後、骨体を正しい位置へ動かさなくてはならない。骨体は、望ましい咬合と一致し、条件を満たす顔面の皮膚表面が得られるように、他の骨格的特徴に関して動かされる。
術中道具
仮想計画立案を手術野に再現するのには、幾つかの手法が可能である。術中道具を備えれば、計画された咬合を患者に再現することができる。恐らくは、頭蓋底に対する(上顎の様な)骨体の正しい位置及び、TMJ窩(TMJは、側頭下顎関節)に対する下顎の上行枝も再現することができるであろう。
術後道具
手術結果を安定させ持続するために、術後スプリントが必要になることもある。更に、この個人に合わせた部品は、計画立案ソフトウェアの結果から製作することもできる。
Claims (19)
- 個人の頭蓋及び歯列の形状情報を引き出すための方法において、
−前記個人の歯列の印象を採得する段階と、
−前記個人が前記印象を装着している状態で、前記個人の頭部の第1の走査画像を撮影する段階と、
−前記印象単独の第2の走査画像を撮影する段階と、
−前記走査画像を組み合わせる段階と、
−前記組み合わされた走査画像から前記形状情報を引き出す段階と、を備えている方法。 - 前記印象を採得する段階は、予め定義された咬合で行われる、請求項1に記載の形状情報を引き出すための方法。
- 前記印象を外した状態の前記個人の頭蓋単独の第3の走査画像を撮影する段階を更に備えている、請求項1又は2に記載の形状情報を引き出すための方法。
- 前記第1の走査画像を撮影する前記段階は、45μSv未満の線量で行われる、請求項3に記載の形状情報を引き出すための方法。
- 前記第3の走査画像は、前記個人の顎顔面複合像を備えている、請求項3又は4に記載の形状情報を引き出すための方法。
- 前記第3の走査画像は、予め定義された咬合と顔の表情で撮影される、請求項3から5までの何れかに記載の形状情報を引き出すための方法。
- 前記第2の走査画像は、前記印象を発泡体状の材料に載せて撮影される、請求項1から6の何れかに記載の形状情報を引き出すための方法。
- 前記印象を採得する前記段階は、印象材料としてアルギン酸塩又はシリコンを用いて行われる、請求項1から7の何れかに記載の形状情報を引き出すための方法。
- 前記印象を採得する前記段階は、ワックスバイトを用いて行われる、請求項1から7までの何れかに記載の形状情報を引き出すための方法。
- 前記個人の歯列の前記印象は、両面印象である、請求項1から9の何れかに記載の形状情報を引き出すための方法。
- 前記印象を採得する前記段階は、CTスキャナで行われる、請求項1から10の何れかに記載の形状情報を引き出すための方法。
- 下顎の自転をコンピュータで計算する段階を更に備えている、請求項1から11の何れかに記載の形状情報を引き出すための方法。
- 骨体を配置し直すために頬顎異常矯正計画立案情報を引き出すための方法において、個人の頭蓋と歯列の情報が、請求項1から12までの何れかに記載の方法で引き出される、方法。
- 計画された咬合を手術野に再現するために術中スプリントを設計及び製作するための方法において、前記スプリントが、請求項1から12までの何れかに記載の方法により引き出された形状情報に基づいて設計される、方法。
- 計画された咬合を再現することができるように、上下の歯列を接続するための固定構造を設計及び製作するための方法において、前記固定構造が、請求項1から12までの何れかに記載の方法により引き出された形状情報に基づいて設計される、方法。
- 前記固定構造は、上下歯列の一組の歯科用ブラケットと、前記ブラケットを一体に接続するための構造と、を備えている、請求項15に記載の固定構造を設計及び製作するための方法。
- 請求項1から12までの何れかに記載の方法により引き出された形状情報によって設計された、特注仕様の骨接合術用プレートを設計及び製作するための方法。
- 手術によって実現された設定状態を持続するための術後維持道具を設計及び製作するための方法において、これにより、請求項1から12までの何れかに記載の方法で引き出された形状情報が利用される、方法。
- プログラム可能な装置上で実行させることのできる、命令を保有しているプログラムにおいて、実行させると、請求項1から18までの何れかに記載の前記方法を行う、プログラム。
