Abstract
Description
本出願は、参照により全体として本明細書に組み込まれる、「METHODS AND DEVICES FOR TREATING STROKE」のタイトルで2016年1月4日に出願された米国仮特許出願第62/274,582号の優先権を主張する。
患者に薬剤を送達するため、および患者から物質を吸引するための方法および装置が、例えば脳卒中の治療のために、本明細書に開示される。
毎年、何百万人もが脳卒中を患っており、脳卒中は、障害および死亡の主な原因となっている。脳卒中により引き起こされる損傷は、早期の有効な治療によって減少され得る。しかしながら、既存の脳卒中の治療は、最適でない場合がある。例えば、1つの治療方法は、圧力を軽減し、脳卒中の結果として脳内に集まった血液を排液するために、患者の脳に脳室ドレーンを挿入することである。組織プラスミノーゲンアクチベーター(tPA)などの血栓溶解剤が、患者に投与されて、血栓を粉砕するのを助け、血栓を排出させることができる。この治療方法は、血栓溶解剤が非常にゆっくり送達されることを必要とし、ドレーンは、長期間(例えば、何時間または何日も)患者の体内にとどまらなければならず、これにより、手術に費やす時間または集中治療室で過ごす時間が長くなり、その結果、最適でない患者の転帰をもたらす。脳卒中を治療するための、改善された方法および装置が引き続き必要とされている。
単一の装置を通じた注入および吸引を可能にする方法および装置が、本明細書に開示される。装置は、血栓の部位まで装置を送達し、それと同時に、または引き続いて、血栓溶解剤もしくは他の薬剤を血栓に注入し、溶解している血栓を患者から吸引することによって、脳卒中を治療するのに使用され得る。この方法および装置は、有利には、より有効な血栓溶解剤の注入および血栓の吸引を可能にすることができる。モジュール式システムも開示され、硬膜下血腫または他の病気を治療する方法も開示される。
単一の装置を通じた注入および吸引を可能にする方法および装置が、本明細書に開示される。装置は、装置を血栓の部位に送達し、それと同時に、または引き続いて、血栓溶解剤または他の薬剤を血栓に注入し、溶解している血栓を患者から吸引することによって、脳卒中を治療するのに使用され得る。方法および装置は、有利には、より有効な血栓溶解剤の注入および血栓の吸引を可能にすることができる。モジュール式システムも開示され、硬膜下血腫または他の病気を治療する方法も開示される。
(1) カテーテルシステムにおいて、

前記TRAシステムは、前記注入カテーテルの遠位端部に配されたハイドロフォンまたはマイクロフォンと、前記ハイドロフォンまたはマイクロフォンから前記注入カテーテルの近位端部まで延びる1つ以上のリード線と、前記ハイドロフォンまたはマイクロフォンによって検出された信号を時間反転させ、時間反転された前記信号に基づいて音波を発するように構成された反射器と、を含む、システム。
前記吸引カテーテルは、前記吸引カテーテルの遠位端部を遠隔操縦するために張力が加えられ得る、第1および第2の操縦ワイヤを含む、システム。
(7) 実施態様1に記載のシステムにおいて、
前記吸引内腔は、C字型横断面をそれぞれが有する、第1および第2の吸引内腔を含む、システム。
(8) 患者を治療する方法において、
カテーテルシステムを前記患者の体内の治療部位まで前進させることと、
前記カテーテルシステムの注入カテーテルを通じて、薬剤および灌注流体のうちの少なくとも一方を前記治療部位に送達することと、
前記カテーテルシステムの吸引カテーテルを、前記注入カテーテルに対して遠位に前進させることと、
前記吸引カテーテルの吸引内腔を通じて、前記治療部位から物質を吸引することと、
を含む、方法。
(9) 実施態様8に記載の方法において、
前記治療部位は、前記患者の脳内の血栓を含む、方法。
(10) 実施態様8に記載の方法において、
前記薬剤は、血栓溶解剤を含む、方法。
吸引された前記物質は、血栓物質を含む、方法。
(12) 実施態様8に記載の方法において、
前記物質を吸引することは、前記吸引カテーテルを前記治療部位内部で操縦することを含む、方法。
(13) 実施態様8に記載の方法において、
前記薬剤を送達することは、音響エネルギーを前記治療部位に加えることによって、前記薬剤の拡散速度を高めることを含む、方法。
(14) 実施態様8に記載の方法において、
前記薬剤を送達することは、音響エネルギーを用いて前記薬剤が分配される方向を制御することを含む、方法。
(15) 実施態様13に記載の方法において、
前記音響エネルギーは、TRAシステムから放出される、方法。
吸引ポートが形成された近位吸引ハウジングを有する吸引カテーテルと、
前記吸引カテーテル内部にスライド可能に配され、注入ポートが形成された近位注入ハウジングを有する注入カテーテルと、
前記吸引ハウジングおよび前記注入ハウジングのいずれかに選択的に連結されるように構成されたキャップハウジングと、
を含む、モジュール式カテーテルシステム。
(17) 実施態様16に記載のシステムにおいて、
前記注入ハウジングに選択的に連結されるように構成された近位挿入ハウジングを有する挿入スタイレットをさらに含む、システム。
(18) 実施態様16に記載のシステムにおいて、
前記吸引ハウジングおよび前記注入ハウジングは、カテーテルモジュールを画定し、
前記システムは、前記カテーテルモジュールに選択的に連結されるように構成された手動制御モジュールを含む、システム。
(19) 実施態様18に記載のシステムにおいて、
前記手動制御モジュールは、前記注入カテーテルを前記吸引カテーテルに対して前進または後退させるための制御装置を含む、システム。
(20) 実施態様18に記載のシステムにおいて、
前記手動制御モジュールは、前記吸引カテーテルを操縦するための制御装置を含む、システム。

吸引内腔および少なくとも1つの吸引ポートを有する吸引カテーテルであって、前記吸引カテーテルは、前記注入カテーテルがスライド可能に配されるチャネルを画定する、吸引カテーテルと、
