Abstract
Description
本願は、概して、血液を1つの心室から別の心室に再分配し、心不全(HF)、心筋梗塞(MI)および肺動脈高血圧症(PAH)等の病態に対処するための経皮的に設置されたインプラントおよび方法に関する。
心不全は、心拍出量が、身体の必要性を満たすために不十分であるか、またはより高い充満圧力においてのみそれを満たす、生理学的状態である。心筋梗塞、冠動脈疾患、弁膜疾患、高血圧症、および心筋炎を含む、HFの多くの根本原因が存在する。慢性心不全は、神経ホルモン活性化および自律神経調節における改変と関連付けられる。これらの補償神経ホルモン機構は、正常な生理学的状況下における、心臓のための大切な支援を提供するが、それらはまた、HFの発症および後続の進行において基礎的役割を果たす。

以前に公知の心房間シャントの前述の短所に照らして、本発明の原理に従って構築されたシャントは、長期間にわたって、管腔開存性を維持する、より耐久性のある構成を提供する。本発明のシャントはさらに、心房間血液体積および圧力非平衡の再配分を可能にしながら、右心房から左心房へのシャントを通して移動する塞栓によって生じる、奇異性塞栓症のリスクを低減させる。
心房間血液体積を再配分させ、左心房圧を低減させるための心房間シャントが、提供され、これは、心不全(HF)または左心房圧の上昇と関連付けられた他の障害を患っている対象を治療する際に有利であり得る。本発明のデバイスの好ましい実施形態は、砂時計または「ディアボロ」形状のステントまたはフレームであり得る、アンカと、フレームを合成生体適合性材料内に封入することによって形成される、導管とを含む。シャントは、心房中隔、好ましくは、卵円窩内に形成される通路内に固着して入れられ、左心房内の血圧が右側の血圧を超えると、左心房から右心房への一方向血流を提供するように構成される。
Claims (19)
- 患者の左心房と患者の右心房との間の心房間中隔内への設置のために構成される、血液体積配分を調整するためのデバイスであって、
第1の領域と、第2の領域と、前記第1の領域を前記第2の領域に継合し、前記心房間中隔に係合するように構成される、縮径領域とを有する、アンカであって、前記アンカは、10〜15mmの範囲内の長さを有し、収縮された送達状態から、前記第1の領域が前記患者の左心房の中に延在する、拡張された展開状態に遷移するように構成され、前記第2の領域は、前記第2の領域の出口が、下大静脈から前記患者の右心房の中に進入する血液の天然循環流路外に位置し、それによって、前記下大静脈からの流動内に同伴される塞栓が、前記第2の領域の出口の中に指向されないように、前記患者の右心房の中に少なくとも5mm延在する、アンカと、
