本開示は、概して、1つ又は複数の骨プレート及びねじを含む骨プレート、システム、及びキット、並びに1つ又は複数の骨プレート及び対応するねじを使用する方法に関する。例示的な用途では、本開示による骨プレート及びねじシステムは、骨位置合わせ、骨切り術、固定処置、骨折修復、及び/又は1つ以上の骨が所望の位置に設定される他の処置等の外科処置中の1つ以上の骨の内部固定に有用であり得る。そのような処置は、骨が、人体の解剖学的構造の他の部分の骨と比較して比較的小さい、例えば、足又は手の骨(例えば、関節又は単一の骨の異なる部分によって分離された隣接する骨)に対して実施され得る。一例では、外反母趾矯正など、中足骨(例えば、第1の中足骨)と楔状骨(例えば、内側楔状骨)との間の位置合わせを矯正するために、骨プレート及びねじシステムの一実施形態を利用する処置を行うことができる。このような処置の例は、ラピダス法である。別の例では、処置は、中足骨(例えば、第1中足骨)の位置合わせを修正することによって行うことができる。このような処置の例は、基底中足骨の骨切処置である。

- 前記骨プレートは、前記第1の固定孔と前記骨プレートの第1の末端部との間に配置された第3の固定孔と、前記第2の固定孔と前記骨プレートの第2の末端部との間に配置された第4の固定孔とを更に備える、請求項28に記載の方法。
- 前記第1の固定孔、前記第2の固定孔、前記第3の固定孔、及び前記第4の固定孔は、前記骨プレートの前記長手方向軸に沿って互いに同軸である、請求項29に記載の方法。
- 前記骨プレートは、Y字形、L字形、T字形、及びU字形のうちの少なくとも1つを画定するように、前記骨プレートの前記長手方向軸から外向きに延在する少なくとも1つの分岐を更に備える、請求項28から30のいずれか一項に記載の方法。
- 前記第1の固定孔を通して前記係止ねじを挿入する前に、
駆動ピンを前記第1の固定孔を通して前記第1の固定孔の下の前記第1の骨部分にねじ込むことであって、前記駆動ピンは、近位端から遠位端まで延在し、前記駆動ピンは、前記遠位端に隣接するねじ切りと、前記遠位端から近位に離間された前記第1の固定孔の断面サイズよりも大きい断面サイズを有する拡大領域とを含む、ことを更に含み、
前記駆動ピンを前記第1の固定孔を通してねじ込むことは、少なくとも前記拡大領域が前記骨プレートの上側を押圧し、それによって前記骨プレートの前記屈曲部を前記分離部に向かって変形させるまで、前記第1の固定孔を通して前記第1の固定孔の下の前記第1の骨部分に前記駆動ピンをねじ込むことを含む、請求項14から31のいずれか一項に記載の方法。 - 前記骨プレートは、長手方向軸を画定する長さを有し、前記第1の固定孔は前記第2の固定孔から離間されており、前記第1の固定孔は前記分離部の一方の側にあり、前記第2の固定孔は前記分離部の他方の側にあり、前記骨プレートは、前記第1の固定孔と前記骨プレートの第1の末端部との間に配置された第3の固定孔と、前記第2の固定孔と前記骨プレートの第2の末端部との間に配置された第4の固定孔とを更に備える、請求項14から32のいずれか一項に記載の方法。
- 前記第1の固定孔を通して前記第1の骨部分に第1の駆動ピンを挿入することと、
前記第2の固定孔を通して前記第2の骨部分に第2の駆動ピンを挿入することと、
前記第3の固定孔を通して前記第1の骨部分に第3の駆動ピンを挿入することと、
前記係止ねじを前記第1の固定孔に挿入する前に、前記第1の駆動ピンを前記第1の固定孔から取り外すことと、を更に含み、前記第1の固定孔を通して前記係止ねじを挿入することは、前記第2の固定孔を通して前記第2の駆動ピンが挿入され、前記第3の固定孔を通して前記第3の駆動ピンが挿入された状態で、前記第1の固定孔を通して前記係止ねじを挿入すること、
を含む、請求項33に記載の方法。 - 前記係止ねじは、第1の係止ねじを備え、第2の係止ねじを更に備え、前記第2の係止ねじは、頭部ねじ山を有する頭部と、シャフトねじ山を有するシャフトとを有し、前記第2の係止ねじは、1.0より大きい圧縮比を有し、前記第2の固定孔は、前記第2の係止ねじの前記頭部ねじ山と係合するためのねじ切りを含み、
前記第2の固定孔を通して前記第2の係止ねじを挿入し、前記第2の係止ねじの前記頭部ねじ山が前記第2の固定孔によって画定された前記ねじ切りと係合するまで、前記第2の係止ねじを前記第2の固定孔の下の前記第2の骨部分にねじ込むことと、
前記第1の係止ねじ及び前記第2の係止ねじを挿入した後、前記第3の固定孔を通して第3のねじを前記第1の骨部分内に挿入し、前記第4の固定孔を通して第4のねじを前記第2の骨部分内に挿入することと、を更に含む、請求項33又は34に記載の方法。 - 前記第1の係止ねじ及び前記第2の係止ねじの前記圧縮比は、各々少なくとも1.5であり、
前記第3のねじ及び前記第4のねじの各々は、1.5未満の圧縮比を示す非係止ねじ又は係止ねじである、
請求項35に記載の方法。 - 前記第1の骨部分は中足骨又は楔状骨であり、前記第2の骨部分は前記中足骨又は楔状骨の他方であり、前記関節は足根中足関節であり、前記足根中足関節にわたって前記骨プレートを配置する前に、
前記中足骨の端部を調製することと、
前記楔状骨の端部を調製することと、
前記中足骨と隣接する中足骨との間の中足骨間角度を閉じるために、前記中足骨を少なくとも前記横断面内で移動させることと、を更に含む、請求項14から36のいずれか一項に記載の方法。 - 前記足根中足関節にわたって前記骨プレートを配置することは、前記中足骨及び前記楔状骨の背側及び内側のうちの少なくとも1つに前記骨プレートを配置することを含む、請求項37に記載の方法。
- 前記中足骨は第1の中足骨であり、前記楔状骨は内側楔状骨である、請求項37又は38に記載の方法。
- 前記足根中足関節にわたって前記骨プレートを配置することは、前記足根中足関節の背側に第1の骨プレートを配置することと、前記足根中足関節の内側に第2の骨プレートを配置することとを含み、前記第1の骨プレート及び前記第2の骨プレートは各々、前記足根中足関節の一方の側の前記第1の固定孔と、前記足根中足関節の他方の側の前記第2の固定孔と、前記第1の固定孔と前記骨プレートの第1の末端部との間の第3の固定孔と、前記第2の固定孔と前記骨プレートの第2の末端部との間に配置された第4の固定孔とを備える、請求項37から39のいずれか一項に記載の方法。
- 前記第1の骨プレートは、前記第1の骨プレートの前記第1の固定孔及び前記第2の固定孔のうちの一方に前記係止ねじを挿入し、前記第1の骨プレートの前記第1の固定孔及び前記第2の固定孔のうちの他方に二次ねじを挿入することによって設置され、
前記第1の骨プレートの前記第3の固定孔及び前記第4の固定孔を通してねじを挿入する前に、前記第2の骨プレートは、前記第2の骨プレートの前記第1の固定孔及び前記第2の固定孔のうちの一方を通して前記係止ねじを挿入し、前記第2の骨プレートの前記第1の固定孔及び前記第2の固定孔のうちの他方を通して二次ねじを挿入することによって設置される、
請求項40に記載の方法。 - 方法であって、
第1の固定孔、第2の固定孔、及び前記第1の固定孔と前記第2の固定孔との間の屈曲部を有する骨プレートを、前記骨プレートを前記屈曲部に対して平坦なプロファイルに向かって曲げることによって張力をかけ、それによって張力をかけられた骨プレートを提供することと、
中足骨を楔状骨から分離する足根中足関節にわたって前記張力をかけられた骨プレートを配置することと、
