Description
第2内腔5と近位バルーン8との間の気体流を可能にする半径方向開口部18を有する。その結果、近位バルーン8の内部は、開口部5a(図2)に接続されるので、近位バルーン8を、開口部5aに入る吹込み空気または別の気体により膨張させることができる。
患者の口または鼻を通して遠位先端を患者の食道に入れ、治療要件に応じて先端が患者の胃、十二指腸または空腸に達するまで、カテーテル1を前進させる。遠位バルーン6の軸方向位置は、遠位バルーンが胃(または十二指腸または空腸)の中に配置されるようにし、一方、カテーテル1の本体にある近位バルーン8の軸方向位置は、患者の胸部に配置されるようにする。カテーテル1の移動は、X線不透過性マークのリング9、X線不透過性のストライプ10および/または患者外側の可視的な挿入マーク11を数えることにより監視する。
患者の口または鼻を通して遠位先端を患者の食道に入れ、治療要件に応じて先端が患者の胃、十二指腸または空腸に達するまで、カテーテル1を前進させる。遠位バルーン6の軸方向位置は、遠位バルーンおよび遮断バルーン33が胃の中に配置されるようにする。カテーテル1の動きは、可視的な挿入マーク11を数えることにより監視して、遮断バルーンが胃に入ったことを確認する。次いで、遮断バルーン33を水で満たし、遮断バルーン33が患者の胃の噴門に接するまでカテーテルを後退させて、胃に対してカテーテルが正確に配置され、バルーン8が患者の胸部に配置されるのを確実にする。更に、遮断バルーンは、カテーテル1がその位置から引き上げられるのを防ぐ。この遮断バルーンは、X線画像を利用せずにカテーテルの正確で、かつ正しい配置を支援するのに非常に有用である。
カテーテル1のその他の操作は、第1実施の形態のカテーテルについての説明と同一なので省略する。
Claims (11)
- 患者の胃、十二指腸または空腸等の、患者の消化管に流動栄養物を運ぶための経腸フィードカテーテル(1)を制御するためのコンピューターシステムであって、
前記経腸フィードカテーテル(1)における少なくとも一つのバルーン(6、8)内部の気体容積Vを増減するためのポンプ(26)を駆動するポンプ駆動手段(27)に、前記コンピューターシステムを接続するのに適した第1接続手段と、
前記少なくとも一つの前記バルーン(6、8)内の圧力pを測定するための圧力計(24)に、前記コンピューターシステムを接続するのに適した第2接続手段と、
容積Vに関する圧力pの数学的導関数dp/dVを計算するための計算手段と、
初期設定のために、前記容積Vに関する圧力pの数学的導関数dp/dVがゼロになるか、または可能な限りゼロに近くなるように、前記バルーン(6、8)内部の気体容積を調整するよう前記ポンプ駆動手段(27)を制御することを前記コンピューターシステムに行わせるための実行命令にアクセスするためのアクセス手段と
を備えることを特徴とするコンピューターシステム。 - 前記圧力計(24)を初期設定のために大気に接続し、また、それを大気から遮断するように構成されたバルブ手段(22、25)に、前記コンピューターシステムを接続するのに適した第3接続手段を更に備え、
前記アクセス手段が、前記コンピューターシステムに、前記圧力計(24)を大気圧に接続させるとともに、前記圧力計をゼロに調整させるようにする命令にアクセスする請求項1のコンピューターシステム。 - 前記容積Vに関する圧力pの数学的導関数dp/dVが、特定の圧力で特定の上限閾値を越える場合、カテーテル内腔の遮断または閉塞を警告する警告手段を更に備える請求項1または2のコンピューターシステム。
- 前記容積Vに関する圧力pの数学的導関数dp/dVが、特定圧力での下限閾値に達しない場合、漏洩を警告する警告手段を更に備える請求項1乃至3の何れかのコンピューターシステム。
- 前記経腸フィードカテーテル(1)は、患者の食道を通って前進するよう適合しており、
前記経腸フィードカテーテル(1)は、近位端(P)、遠位端(D)、および第1内腔(2)を有し、
前記第1内腔(2)は、供給源から前記栄養物を受け取り、前記近位端(P)から前記遠位端(D)まで前記栄養物を運ぶために、前記供給源に接続するよう適合している開口部(3)をその近位端(P)に有し、
前記第1内腔(2)の前記遠位端(D)には、患者の消化管に前記栄養物を排出するための少なくとも一つの出口(4)が設けられ、
前記経腸フィードカテーテル(1)は、その近位端で、気体のソース/シンク(26)および気体圧力計(24)に接続されるとともに、前記遠位端(D)近傍で、前記カテーテル(1)上に設けられた膨張および収縮する遠位バルーン(6)に接続される第2内腔(7)を更に備えている、
ことを特徴とする請求項1乃至4の何れかのコンピューターシステム。 - 患者の胃、十二指腸または空腸等の、患者の消化管に流動栄養物を運ぶための経腸フィードカテーテル(1)内の圧力測定を実行するための、コンピュータ読み取り可能な記憶媒体に格納された、コンピュータプログラムであって、
初期設定のために、コンピュータに、
前記経腸フィードカテーテル(1)の少なくとも一つのバルーン(6、8)内部の気体容積Vを増減するようポンプ(26)を駆動させるとともに、前記少なくとも一つのバルーン(6、8)内の圧力pの圧力読取り値を収集する手順と、
容積Vに関する圧力pの数学的導関数dp/dVを計算する手順と、
前記容積Vに関する圧力pの数学的導関数dp/dVがゼロとなるか、または可能な限りゼロに近くなるように、前記バルーン(6、8)内部の気体容積を調整する手順と
を実行させることを特徴とするコンピュータプログラム。 - 前記コンピュータに、前記圧力計(24)を大気に接続する手順と、前記圧力計をゼロに調整する手順と、前記圧力計を大気から遮断する手順を更に実行させることからなる請求項6のコンピュータプログラム。
- 容積Vに関する圧力pの数学的導関数dp/dVが、特定の圧力で特定の上限閾値を越える場合、カテーテル内腔が、遮断または閉塞しているという警告を発する警告手段を作動させる手順をコンピュータに更に実行させることからなる請求項6または請求項7のコンピュータプログラム。
- 容積Vに関する圧力pの数学的導関数dp/dVが、特定圧力での下限閾値に達しない場合、カテーテル内腔(6、8)に漏洩があることを警告する警告手段を作動させる手順をコンピュータに更に実行させることからなる請求項6乃至8の何れかのコンピュータプログラム。
- 初期設定を達成した後、圧力読取り値を連続収集する手順を前記コンピュータに更に実行させることからなる請求項6乃至9の何れかのコンピュータプログラム。
- 圧力が特定の閾値を越えた場合、警告を作動させる手順を前記コンピュータに更に実行させることからなる請求項10のコンピュータプログラム。
