Abstract
본 발명은 배란 진단키트의 기능에 요(소변)중의 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C 등의 검사항목을 진단할 수 있는 체외 진단시약을 추가하여, 이 진단키트를 이용하는 여성에게 배란일 예측과 함께 임신에 영향을 미치는 건강지표들을 함께 제공하여 임신 또는 피임을 결정하는 데에 도움을 주도록 한 것이다.
Description
본 발명은 의료약품의 진단 시약 중 체외진단 의약품에 관한 것으로, 특히 요를 이용한 건강지표 진단항목 11가지{적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C}를 배란진단시약과 함께 하나의 키트로 제작하여 한번의 검사로 배란시기 예측과 사용여성의 임신관련 건강정보를 함께 제공할 수 있도록 한 자가 배란 진단 키트에 관한 것이다.
기존의 배란 진단키트는 단지 LH-Surge를 확인하여 배란일을 예측할 수 있도록 구성되어있다. 따라서, 사용자가 임신을 원하여 배란 진단키트를 사용할 경우, 건강상태가 임신에 적절한 상태인지 부적절한 상태인지를 확인할 수 있는 건강지표는 제공하지 못하였다.
따라서, 본 발명에서는 배란 진단키트의 기능에 요의 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등의 검사항목을 진단할 수 있는 체외 진단시약을 추가하여, 이 진단키트를 이용하는 여성에게 배란일 예측과 함께 임신에 영향을 미치는 건강상태에 관한 지표들을 함께 제공하여, 임신 또는 피임을 결정하는 데에 도움을 주려고 하는 것이다.
본 발명의 목적은 건강 지표 검사 부위와 배란진단 부위가 함께 배치되어 있고, 건강 지표 검사 부위는 흡수체 상부에 건강 지표를 검사하는 진단 시약이 일정 면적으로 고정 도포되어 있으며, 배란진단 부위는 흡수체 상부(배란진단표시창)로 요를 이동시키는 이동부, 배란여부를 표시하는 배란진단 표시창, 및 검사 종료를 나타내는 검사종료 표시창이 구성되어 있는, 건강 지표를 진단하고 배란일을 예측할 수 있는 자가 배란 진단키트를 제공하는 것이다.

본 발명은 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등의 검사기능을 포함한 자가 배란 진단키트로서, 본 발명에서는 배란일 예측을 위한 배란 진단키트와 요를 이용한 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등의 검사 항목을 함께 가진 체외 진단키트를 제작하기 위하여, 배란 진단시약과 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C 진단시약을 하나의 키트에 포함되도록 구성하였다.
본 발명의 진단키트에서 사용되는 용어와 진단원리를 설명하면 다음과 같다.
1. 배란 진단의 원리와 배란 진단 키트의 작용원리
배란이란 사람의 뇌하수체 전엽호르몬인 여포자극호르몬(FSH, Follicle Stimulation Hirmone)에 의해 난소의 여포가 성숙된 후, 황체 호르몬(hLH, human luteinizing hormone)의 작용에 의해 여포에서 난자가 방출되는 현상이다.
이 hLH는 월경 주기중 대부분의 시기에는 소량 분비되지만 주기의 중간에 hLH 양이 급격히 증가되며, 이를 LH-Surge라하고, LH-Surge 이후 보통 24-36 시간내에 배란이 일어난다.그리고, hLH는 요로 배출된다.
따라서, 요중의 hLH와 배란 진단키트에 부착된 hLH 단일항체와의 항원-항체 반응을 이용하여 월경주기중의 LH-Surge를 감지함으로써 배란 시기를 예측할 수 있다. 월경 주기중 요중의 hLH 농도는 다음 <표 1>과 같다.

종래의 연구에서 hLH 항체를 생쥐와 염소 등의 세포에서 합성, 정제하여 hLH에 특이적으로 결합하는 단일항체를 획득하였으며, 이를 응용한 배란 진단키트가 제작되어 있다. 이 배란 진단키트는 요의 hLH와 배란 진단키트내에 부착된 hLH 단일항체와의 항원-항체 반응을 이용하여 hLH-Surge를 확인함으로써 배란일(LH-Surge후 24-36시간)을 예측한다.
배란 진단키트는 요흡수부위, 배란진단부위, 연결부위로 구성된다. 요흡수부위성분은 흡수지이며, 연결부위는 유리섬유, 흡수지, 셀루로스지에 쥐의 항 hLH 단일항체-콜로이드성금입자혼합물(Mouse anti hLH monoclonal antibody-colloidal gold conjugates)(임의의 특정한 색을 띠고 있음)(도1b의 5, 도2c의 11)이, 배란진단부위의 배란표시창(도1a 및 도2a의 T)에는 염소의 항 hLH 복합 항체(Goat anti hLH polyclonal antibody)가 도포되어 있으며, 배란진단부위의 검사종료표시창(도1a, 및 도 2a의 C)에는 염소의 항마우스 항체(Goat anti-mouse antibody:IgG)가 도포되어 있다. 이때 쥐의 항 hLH 단일항체-콜로이드성금입자혼합물의 양이 배란표시창에 있는 염소의 항 hLH 복합항체(T)의 양보다 많다. 배란진단키트의 요중 hLH 검출한계는 약 25 mIU/ml로 제작된다.
* hLH-Surge인 경우 즉, 배란 1-1.5일전
요가 흡수 막대를 타고 이동하면서 요속의 hLH가 쥐의 항 hLH 단일항체-콜로이드성금입자혼합물과 결합한다. 이 결합체가 계속해서 요를 따라 이동하다 배란 표시창(T)에서 염소의 항 hLH 복합항체와 결합하여 머무르게되면 임의의 특정한 색이 나타나게 된다. 그리고, 배란 표시창에 붙고 남은 잉여의 쥐의 항 hLH 단일항체-콜로이드성금입자혼합물이 계속 이동하여 검사종료 표시창에 도포되어 있는 염소의 항마우스 항체와 결합하여 머무르게 되면 임의의 특정한 색이 나타나게 되어 사용된 검사시약이 정상적으로 작용되었는지를 확인해준다. 따라서, 배란 표시창과 검사 종료 표시창에 각각 하나씩, 두개의 선이 나타난다.
* hLH-Surge가 아닌 경우
요가 흡수 막대를 타고 이동하며, 이 경우에는 요중에 hLH 농도가 매우 낮으므로 쥐의 항 hLH 단일항체-콜로이드성금입자혼합물이 배란 표시창의 염소의 항 hHL 복합항체와 결합이 일어나지 않아 그냥 지나가게 되고, 검사종료 표시창에 도포된 염소의 항 마우스 항체와만 결합하여 임의의 특정한 색이 나타나게 된다. 따라서 검사종료 표시창에만 선이 나타난다.
2. 요를 이용한 건강지표의 검사원리
1)적혈구 검사: 적혈구 헤모글로빈에서 유리된 햄(heme)의 페록시다아제 (peroxidase)의 활성을 이용한다. 적혈구 헤모글로빈의 페록시다아제활성은 3.3, 5.5-테트라메틸벤지딘, 2.5 디메틸 핵산-2.5-디하이드로퍼옥시드 그리고 organic peroxide의 반응을 촉진한다. 그결과 요의 적혈구 양에 비례하여 시약의 색을 greenish-yellow에서 dark blue로 변화시킨다.
2) 백혈구 검사: 백혈구의 대부분인 호중구 내의 에스테라제의 활성을 응용한다. 적혈구의 에스테라제는 티아졸 아미노산 에스테르에 작용하여 이를 하이드록시 티아졸로 전환시키며, 하이드록시 티아졸은 디아조늄 염과 반응하여 보라색을 나타낸다. 즉 요증 적혈구의 양과 보라색의 농도는 비례한다.
3) 단백질 검사: 테트라브로모페놀 블루가 단백질과 반응하여 변색(황색에서 녹색-청녹색으로 변화)되는 성질을 이용한다.
4) 포도당 검사: 2단계의 효소반응을 이용한다. 먼저 포도당 산화효소(glucose oxidase)가 포도당을 분해하여 글루콘산과 과산화수소를 형성하고, 과산화 효소(peroxidase)가 과산화수소를 요오드화 칼륨(potassium iodate chromogen)과 반응시켜 크로모겐을 산화시켜 색을 청색 또는 녹갈색에서 암갈색으로 바꾼다.
5) 수소이온농도 검사: 메틸레드와 브로모티몰 블루의 수소 이온 농도 변화에 따른 색 변화(오렌지, 황녹색, 또는 녹색에서 청색으로 변함)를 이용한다. 6) 비중 검사: Polyelectrolytes의 이온화 농도와 관련한 pka 변화에 따른 양이온과 (-COOH)n 과의 반응에 의해 H+가 방출되는데 이 H+가 수소이온농도를 표시하는 지시약과 반응하여 색의 변화를 일으킨다.
7) 유로빌리노겐 검사: 4-메톡시벤조데아조늄과 소변의 유로빌리노겐이 강한 산성의 조건에서 디아조늄 반응으로 색이 핑크 빛에서 적갈색으로 변화됨을 이용한다.
8) 빌리루빈 검사: 요중의 빌리루빈이 디아조니움염과 coupling반응을 일으켜 지시약을 갈색에서 보라색으로 변화시킨다.
9) 케톤체 검사: 요중의 아세토아세틱에시드와 니트로프루시드와의 반응에 의하여 지시약의 색이 갈색에서 보라색으로 변화한다.
10)아질산염 검사: 아르사닐릭에시드와 요중의 아질산염(nitrite)와 반응하여 디아조니움을 형성하며, 이 디아조니움이 N-(1-naphthyl)ethylen??diamine 또는 3-히드록시 1,2,3,4-테트라히드로-벤조(h)퀴놀린과 반응하여 지시약의 색변화를 일으킨다.
11)비타민 C: 아스코빅 에시드와 2,6-디클로로페놀 인도페놀 소디움이 반응하여 pH의 변화를 일으켜 pH 지시약인 메칠레드의 색을 변화시킨다.
이러한 원리를 이용한 체외 진단 시약들이 제조되어 사용되고 있다.
3. 건강지표들의 임상적 의의
본 발명에서 사용된 건강지표들의 임상적 의의는 <표2>와 같다.

이하, 본 발명의 일 예를 도면과 관련하여 구체적으로 설명하고자 한다.
이러한 키트는 도 1에 나타난 제 1 형과 도 2에 나타난 제 2 형으로 구성될 수 있다. 도 1a에서 건강 지표 검사부위(1)은 요를 이용한 임신과 관련된 건강지표들을 검사하는 부위로서, 플라스틱 판위에 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등의 양을 검사할 수 있는 진단시약들이 부착되어 있다. (2)는 요를 이용하여 배란일을 예측하기 위한 배란 진단 부위중 요를 진단시약에 적시기 위하여 구멍이 뚫어져 있다. (T)는 LH-Surge를 확인하기 위한 시험부위 표시이며, (C)는 배란 진단시약의 정상적인 반응을 확인하기 위한 대조시험 부위(검사종료 표시창)이다. (6)은 진단시약들을 지지하는 지지체로서 플라스틱 재질로 되어 있으며, 윗면과 아랫면으로 구성되어 있다. 도 1b에서, (3)은 건강지표 검사부위(1)밑에 깔려진 흡수지이며, (4)는 배란진단시약 부위이다. (5)는 생쥐 항 hLH 단일항체-콜로이드성금입자혼합물, (T)는 염소 항 hLH 복합항체, (C)는 염소 항 생쥐 항체(Immunoglobulin G:IgG)이며, 이들 성분들은 (4)(유리성 섬유, 셀루로스지와 흡수지로 구성)에 부착 또는 뿌려져 있다.
도 2a에서 (7)과 (8)는 키트를 구성하는 주요 몸체로서 분리가 가능한 구조이다. (8)는 2개의 플라스틱 구조물(윗면과 아랫면)이 결합된 상태이다. (9)는 요를 이용하여 배란일을 예측하기 위한 배란 진단부위의 표시창이다. (T)와 (C)는 도1에서 기술된 부위와 같은 기능을 갖는다.
도 2b의(10)은 도2a의 7을 제거한 후 나타나는, 도1에서의 (1)과 같은 건강지표 검사부위이다. (10)의 뒷부분에는 배란진단 부위에 요를 반응시키기 위한 요 흡수막대(유리섬유, 셀룰로스지와 흡수지로 구성)가 위치한다. 도2c의 11은 도1b의 5와 같은 성분이다.

상기의 시약 성분 중 배란진단부위의 경우 각각의 성분이 1 mm × 3 mm 평면 범위에 고정도포 또는 뿌려져 있다. 생쥐의 항 hLH 단일항체-콜로이드성금입자혼합물의 경우 요를 따라 이동이 가능하도록 뿌려져 있으며, 염소의 항 hLH 복합항체와 염소의 항 생쥐 항체(IgG)는 고정 도포 되어 있다.
그리고, 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등의 검사성분들도 3mm × 3mm의 평면 부위에 각각 고정 도포되어 있다.
적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등을 진단할 수 있는 진단시약의 성분 범위는 본 명세서에 제시된 진단시약 성분 이외에 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C등을 요를 이용하여 진단할 수 있는 기타 시약들을 포함하며, 배란 진단 시약의 범위도 본 명세서에 제시된 생쥐 항 hLH 단일항체-콜로이드성금입자혼합물과 염소 항 hLH 복합항체, 염소 항 생쥐 항체(IgG)이외에 동일한 목적의 단일 또는 복합 항체를 다른 실험동물에서 합성한 경우를 포함한다.
본 발명의 구체적인 적용 예를 보면,
예1) 배란진단 부위에 hLH 양성 반응을 보이고 요의 유로빌리노겐 농도가 높게 나타날 경우 간 기능 장애, 빈혈 또는 항생 물질 투여 등이 의심되므로 임신 생활에 장애를 유발할 가능성이 있다. 따라서, 전문병/의원을 방문하여 진단 후 임신 또는 피임을 결정한다.
예2) 배란진단부위에 hLH 양성 반응을 보이고 요의 포도당 농도가 높게 나타날 경우 당뇨병이 의심되며, 당뇨는 임신 생활에 장애를 유발할 가능성이 있다. 따라서, 전문병/의원을 방문하여 진단 후 임신 또는 피임을 결정한다.
예3) 배란 진단부위에 hLH 양성 반응을 보이고 요의 모든 항목이 정상범위이면 임신을 결정하는데 도움을 준다.
본 발명의 효과는 사용자의 배란일 예측과 함께 임신에 영향을 미칠 수 있는 건강 지표들을 함께 진단하여, 사용자의 건강상태가 임신에 부적절한 경우 전문병/의원의 방문을 유도하여 진단을 받음으로써 사용자가 임신 또는 피임을 결정하는 데에 도움을 준다.
Claims (5)
- 건강 지표 검사 부위와 배란진단 부위가 함께 배치되어 있고,건강 지표 검사 부위는 흡수체 상부에 건강 지표를 검사하는 진단 시약이 일정 면적으로 고정 도포되어 있으며,배란진단 부위는 흡수체 상부로 요를 이동시키는 이동부, 배란여부를 표시하는 배란 표시창, 및 검사 종료를 나타내는 검사종료 표시창이 구성되어 있는 건강 지표를 진단하고 배란일을 예측할 수 있는 자가 배란 진단키트.
- 제 1 항에 있어서, 건강 지표 검사 부위가 배란 진단 부위 상단에 위치한 자가 배란 진단키트.
- 제 1 항에 있어서, 건강 지표 검사 부위가 배란 진단 부위와 횡렬로 위치한 자가 배란 진단키트.
- 제 1 항에 있어서, 건강 지표 검사 부위가 다수의 건강 지표를 진단할 수 있도록 다수의 진단 시약이 일정 간격으로 고정도포 되어 있는 자가 배란 진단키트.
- 제 4 항에 있어서, 진단 시약이 적혈구(RBC), 백혈구(WBC), 단백질, 포도당, 수소이온농도(pH), 비중, 유로빌리노겐, 빌리루빈, 케톤체, 아질산염, 비타민 C를 검사하는 시약인 자가 배란 진단키트.
