Abstract

A device comprising a tubular member (10, 20) for insertion into the distal end of the urethra (4), and manually operated sealing means in an external end portion (11, 21) of the tubular member which remains outside the penis. The tubular member includes an intermediate portion forming a flexible balloon (13, 23) spaced from the external end portion so that it may be placed and retained in the navicular fossa (2) when said external end portion is in abutment with the tip of the penis. The device is useful for treating male urinary incontinence.

Cette fossette naviculaire est un élargissement fuselé de l'urètre terminal situé à une très courte distance du méat urinaire . Le blocage en position du dispositif de continence urinaire est assuré par le gonflement du ballonnet dont la paroi vient s'appliquer contre celle de la fossette naviculaire en se moulant en quelque sorte dans cette dernière. Il s'ensuit que le maintien en position est assuré sans distension notable de la paroi de l'urètre, le ballonnet une fois gonflé ne pouvant cependant pas ressortir de son logement dans la dite fossette naviculaire du fait du rétrécissement de l'urètre au niveau du méat urinaire. By way of example, the wall of the balloon may be of the order of 0.5 to 1.5 mm, for a diameter of the tubular element of approximately 5 mm; the section of the reinforcing strips 55 being for example 1.5 mm wide by 2 to 3/10 mm thick.
Les figures 9 et 10 illustrent une variante de réalisation des moyens d'obturation. Dans cette variante, la partie d'extrémité externe 51 comporte une sorte d'agrafe, formée d'un fil élastique, qui comporte deux lobes 71, 72 qui entourent chacun l'orifice 73 du dispositif . Dans la position de fermeture représentée figure 9, la conformation initiale de l'agrafe et l'élasticité de la liaison 74 entre les lobes tendent à éloigner les deux lobes l'un de l'autre, assurant le pincement de la paroi de l'orifice et son obturation. Pour la miction, il suffit de presser diamétralement la dite partie d'extrémité 51, comme représenté par les flèches F2 , ce qui ramène les lobes 71, 72 dans la position représentée figure 10, et provoque l'ouverture de l'orifice 73. Le relâchement de la pression ramène l'obturateur dans la position de la figure 9, sous l'effet de rappel élastique de l'agrafe 70. Le fonctionnement et la réalisation très simples de cet obturateur est rendu possible grâce à la grande souplesse du matériau utilisé pour le dispositif. En relation avec la figure 6, on va maintenant décrire la mise en place du dispositif, au moyen d'un mandrin 60. Tel que représenté sur la figure, le dispositif forme avec le mandrin en ensemble préparé, commercialisable tel quel. Le dispositif est placé sur le mandrin 60 en matière rigide ou semi rigide. Le mandrin 60 comporte un épaulement 61 adapté pour venir buter contre la bague 56 et son extrémité 62, qui dépasse légèrement au delà de la lèvre 54 , est arrondie pour faciliter son introduction dans l'urètre. Il comporte une chambre 63 qui s'étend axialement et se termine du coté extérieur, à droite sur la figure, dans une partie cassable 65 reliée au corps du mandrin par une amorce de rupture 66. A l'autre extrémité, des orifices 64 répartis circonférentiellement débouchent d'une part dans la dite chambre et d'autre part à la périphérie du mandrin au niveau de la lèvre 54. Une dépression est créée dans la chambre 63 lors de la fabrication, cette dépression étant suffisante pour maintenir la lèvre 54 plaquée autour du mandrin comme représentée figure 6 et 7. On notera que dans cette position, la surface extérieure de la lèvre forme un cône incurvé sensiblement dans le prolongement de l'arrondi de l'extrémité 62 du mandrin, de manière à faciliter l'introduction dans l'urètre sans blesser la muqueuse du conduit urinaire.Figures 9 and 10 illustrate an alternative embodiment of the sealing means.
