Abstract
Description
低侵襲性の(内視鏡)手術中に作業空間を拡張、再構成(再変形)する為に、安定した最適な作業(処置)空間を設けることは有利である。そのような安定して拡張され、最適に構成された作業空間により、器具や内視鏡は個別に操作しやすくなり、標的組織の内視鏡による視認性も改善される。
本明細書に説明するシステムは、組織を撤収する為のほぼ密封されたチャンバーも形成する。
本願のシステムの別の実施形態では、システムは、カテーテル内に浮動(可撓性の)チャンネルを備える。一実施形態では、浮動チャンネルは、近位端と遠位端で固定されている。別の実施形態では、浮動チャンネルは、近位端で固定されているが、遠位端では固定されていない。浮動チャンネルは、カテーテル(外部チューブ)の全体の剛性を減弱する。もしチャンネルが全長に沿って固定されていてカテーテル内に浮動していない場合には、カテーテルはより剛性が高いであろう。浮動チャンネルは、浮動チャンネルの中に挿入される器具チャンネル(可撓性ガイド)のねじれ(kinking)や器具チャンネルの中に挿入される器具(または、器具チャンネルが使用されない実施形態では浮動チャンネルの中に直接挿入される器具)のねじれも抑制する。使用できる浮動チャンネルは、2015年2月14日付の特許出願第14、622,831号明細書(公報第2015/0157192号明細書)に記載されており、その全体の内容は、引用によりここに援用する。
使用時には、器具は、ウインドウを通過してチャンバー内に入る組織にアクセスしたり、ウインドウの外側の組織にアクセスする為にウインドウを通過して突出できる。
Claims (15)
- 患者の体管腔内における内視鏡手術用システムは、
近位部と、遠位部と、前記遠位部において拡張可能なバルーンとを有する可撓性カテーテルを備え、
前記拡張可能なバルーンが側方アクセス開口部を備えることと、前記拡張可能なバルーンが収縮時挿入形態から拡張時形態に拡張する為に膨張可能であることと、前記拡張可能なバルーンが、前記カテーテルの第1の側に拡張したチャンバーを形成するために非対称的に拡張して拡大した横断面を備えることと、
前記側方アクセス開口部が、標的組織にアクセスする側方ウインドウを形成する為に前記カテーテルの第1の側に配置されることと、
前記カテーテルは、内視鏡器具を内部に受承する寸法に形成された管腔を備え、前記内視鏡器具の遠位端は、前記拡張したチャンバー内に配置されて、前記拡張したチャンバー内で横方向に曲がって前記側方ウインドウを通過して標的組織にアクセスすることと、
前記カテーテルの管腔が、前記チャンバーに連通する遠位端に開口部を備えることと、
前記カテーテルは、標的組織を可視化する為に可視化装置を受承する寸法に形成された管腔をさらに備えることと、
前記カテーテルの管腔に受承された内視鏡器具が、前記可視化装置とは別個に移動可能であることと、
からなるシステム。 - 前記カテーテルは、前記バルーンを拡張する為の膨張チャンネルを備える、請求項1に記載のシステム。
- 前記拡張可能なバルーンは、その長さに沿って一様な横断面を有する、請求項1または2に記載のシステム。
- 前記拡張可能なバルーンは、C字形状の横断面を有する、請求項1または2に記載のシステム。
- 前記カテーテルの長軸に対して前記カテーテルの遠位部を曲げる為の湾曲部材をさらに備える、請求項1〜4のいずれか一項に記載のシステム。
- 前記拡張可能なバルーンは、前記チャンバーの終端を形成する、請求項1〜5のいずれか一項に記載のシステム。
- 前記拡張可能なバルーンは、U字形状内に空間を形成するU字形状を有する、請求項1〜6のいずれか一項に記載のシステム。
- 前記内視鏡器具は、第1の方向に延びる第1の湾曲部と、反対の方向に延びる第2の湾曲部とを有し、第1の湾曲部は、前記側方ウインドウから離間する方向に曲がって進み、且つ第2の湾曲部は、前記側方ウインドウに向かって曲がって進む、請求項1〜7のいずれか一項に記載のシステム。
- 前記カテーテルは、遠位端にシールを備え、前記可視化装置は、前記カテーテルの遠位方向を可視化するために前記シールを通過して移動できる、請求項1〜8のいずれか一項に記載のシステム。
- 前記カテーテルの管腔内に配置される可撓性ガイドをさらに備え、前記可撓性ガイドは、前記拡張可能なチャンバーの中に進行する遠位部を備え、前記内視鏡器具は、前記可撓性ガイドの内部に挿入可能である、請求項1〜9のいずれか一項に記載のシステム。
- 前記拡張可能な部材の中間部は、近位部と遠位部よりも大きな横断面を有し、前記側方ウインドウは、前記中間部に配置される、請求項1〜10のいずれか一項に記載のシステム。
- 患者の体管腔内における内視鏡手術用の可撓性カテーテルは、近位部と、収縮時挿入形態から拡張時形態に拡張できる拡張可能なバルーンを有する遠位部とを備え、
前記拡張可能なバルーンは、前記カテーテルの第1の側に拡張したチャンバーを形成するために、非対称的に拡張して拡大した横断面を備え、
前記拡張可能なバルーンは、U字形内の空間が標的組織にアクセスする開口部を形成する、前記U字形を備え、
前記カテーテルは、前記拡張したチャンバーに連通する遠位開口部を有する管腔を備える、カテーテル。 - 前記拡張可能なバルーンは、近位部と遠位部より大きな横断面を有する中間部を備え、前記空間は、前記中間部に配置される、請求項12に記載のカテーテル。
- 前記拡張可能なバルーンは、前記チャンバーの終端を形成する、請求項12または13に記載のカテーテル。
- 前記カテーテルは、最も遠位部にシールを備える、請求項12〜14のいずれか一項に記載のカテーテル。
